Transferring Paperwork

Patient care may be initiated prior to the full transfer of medical records. A medical records release form will be completed during the initial visit.


Please ensure that you bring your child’s current insurance card, along with a valid photo identification for the parent or legal guardian.

Please be advised that under federal (HIPAA) and state regulations, your child’s previous medical provider may charge a reasonable, cost-based fee for copying medical records. This fee is the responsibility of the parent or guardian.


In some cases, this fee may be waived if the records are sent directly to our office.

At iCare Pediatrics, we will establish care during your child’s first visit, and at that time, we will request medical records from any previous providers. These records help us better understand your child’s health background, vaccinations, and any past treatments.



If you have copies of your child’s records or immunization history, feel free to bring them to the first appointment. Our team is happy to assist in coordinating the transfer of medical information to ensure a smooth transition of care.

When transferring your child to iCare Pediatrics  , it's important to ensure a smooth and seamless transition by taking the following steps:


  • Gather and organize all medical records, including immunization records, growth charts, and any relevant lab or test results
  • Communicate with both your current and new pediatric providers to coordinate the transfer of care
  • Be prepared to provide details about your child’s medical history, past treatments, allergies, and ongoing medications
